Montgomery County, Ohio birth certificates are filed with the Bureau of Vital Statistics.
Applying for a home loan, a passport and many other items requires you to have a copy of your birth certificate in Montgomery County, Ohio. If you can't find a copy of your birth certificate, contact the Bureau of Vital Statistics of Public Health -- Dayton and Montgomery County.
Instructions
By Mail
1. Compose a letter to the Vital Statistics Office requesting a copy of your birth certificate.
2. List the name, date of birth, hospital of birth, gender and the names of any parents listed on your birth certificate in your letter. Provide your current name, mailing address, contact information and your reason for needing a copy of your own birth certificate.
3. Write a check for $22, which is the cost for a copy of your birth certificate as of November 2010. Mail your check, request and a self-addressed, stamped envelope to the Montgomery County Vital Records Office (see Resources).
Your birth certificate will be mailed to you once your request has been received and processed. The Office of Vital Statistics' website states it has limited employees and processes many requests, therefore it could take a number of weeks before your request is fulfilled.

4. Visit the Montgomery County Bureau of Vital Statistics.
5. Speak with a Bureau of Vital Statistics employee about obtaining a copy of your birth certificate. Provide him with all of the information on your birth certificate and wait while your certificate is located.
6. Pay the $22 fee (as of 2010) for your certificate with a check, money order or credit card. Credit card purchases incur an additional $7 processing fee.

7. Visit the VitalChek website (see Resources). The Montgomery County Vital Statistics Office recommends you use this site for any online birth certificate orders.
8. Enter all of the information requested by VitalChek as you are prompted by the site.
9. Indicate a shipping method, enter your credit card and shipping information, and submit your request. Fees vary based on the shipping method you chose.
